Welcome to our Practice!

The practice currently has an open list and welcomes new patient registration requests from people who live within the practice area.

Register with a GP surgery

If you prefer to register in person, our new patient welcome pack is available from reception. We look forward to meeting you when you call into collect the registration forms. If possible, please bring photo identification and proof of address with you when registering, this is incredibly helpful and ensures a smooth transition. Please do not worry if you do not have identification, this is required for registration with the NHS App, which you can do directly at a later date should you prefer.

New Patient Health Check

Newly registered patients will be invited to attend a health check with a healthcare assistant.

This will include measuring height, weight, blood pressure, a urine test and obtaining details of past medical history, family and health problems, thereby recording important health details before the medical records are received.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Patients Rights and Responsibilities

  • Be courteous to our staff at all times, please remember they are trying to help you. We genuinely care about our patients and do our best to meet everyone’s needs.
  • Inform us when you move home or change your name so that our records are correct and up to date. Please don’t forget to let us know if you change your mobile phone number.
  • Be on time for your appointments and check-in at Reception to ensure the clinician can be informed that you have arrived.
  • Let us know if you need to cancel your appointment, this helps us to offer the appointment to another patient.
  • Make separate appointments for each member of the family that needs to be seen, so that we can give each of you time to help you.
  • Let us have your views. Your ideas are important in helping us make our practice a friendly and safe place to visit for medical treatment.

Violent of Abusive Behaviour

Wallingbrook Health Group in line with the NHS operates a Zero Tolerance policy in respect of our staff being verbally or physically threatened or abused. Such aggressive and intimidating behaviour will not be tolerated against any member of our Practice team. Any patient who acts in this manner will be removed from the Practice list immediately.
